Our dental staff will then check if your child is eligible for the program and help you sign a consent form for our services. Once everything is in order, your child can receive our dental care at West Perth Dental Centre. To claim your child’s benefits, you will need to create a MyGov account and link it to Medicare. From there, you can access your payment details and submit claims for reimbursement. You can also mail a filled out Medicare claim form to the address on the form.
